We have new referral cards available at our front desk, or you can just mention the patient's name on a new patient form. 

If you refer a friend to our clinic, you will BOTH receive 10% off your next appointment.

Terms and Conditions

• Only one discount can be used at each appointment

• Can not be used on the New Patient Package

• You may refer more than one friend, and you will receive 10% off for each friend you refer. However, only one discount will be used at each appointment. 

• Your friend must become a paying patient at Gentle Care Dentistry 

• The referred patient must be a new patient at the clinic

• The referred patient must mention your name on the referral card or new patient form to get discounts for both patients. 

• Maximum discount of $150 per referral
